Open Access Services
On Monday, Wednesday, and Friday at 8:00 am we have Open Access services for new clients to receive an evaluation on a walk-in basis. Starting April 1st we will have Open Access for new clients every day, five days a week Monday through Friday starting at 8:00am. Before you arrive, please remember to bring the following documents:
We serve adults with insurance or Medicaid and uninsured at low cost according to an income scale.
Uninsured children and Medicaid insurers like PeachCare, WellCare, Cenpatico, Caresource & Amerigroup are received. WE NOW ACCEPT AMBETTER.
- Proof of address (a recent utility bill, rent receipt, or lease agreement in your name).
- Proof of income (such as a pay stub)
- An identification card (such as a driver’s license or photo ID).
- And if you or your child has Medicaid, please bring your Medicaid card with you.
NOTICE TO PATIENTS:
This practice serves all patients regardless of inability to pay.
Discounts for essential services are offered based on family size and income.
